Filing a Complaint
An individual who wishes to file a complaint against a Licensed Professional Counselor may send for a complaint form:

or call 1-304-733-5494 to request the appropriate form. If you get our voice mail, please leave the following information: your name, address, phone number and request a complaint form. Please speak slowly and spell unusual street names.
