The following information is intended to inform consumers the procedure for filing a complaint against a licensee and  the Board's process in reviewing and investigating complaints.

Filing a Complaint

An individual who wishes to file a complaint against a Licensed Professional Counselor may send for a complaint form:

Request for a Complaint form
Post Office Box 129
Ona, West Virginia 25545

or call 1-800-520-3852 to request the appropriate form.  If you get our voice mail, please leave the following information:  your name, address, phone number and request a complaint form.   Please speak slowly and spell unusual street names. 

or email your request to counselingboard@msn.com.   

or print the form from this website.

Upon receipt of request, a complaint form will be mailed within 3 working days.
